Conflict of Interest Policy Annual Statement of Compliance and Disclosure
I have reviewed the Conflict of Interest Policy of the Board of Tufts University. To the best of my knowledge, I have complied with the policy and will use my best efforts to comply with it in the future. Except as described below, neither I, nor a person or entity closely associated with me, has, to the best of my knowledge, a conflict of interest with Tufts University. As used in the preceding sentence, the term “closely associated” shall have the meaning specified in Section 12.1.2 of the Bylaws of the University. (List here any exceptions to the statement above.)

Subsequent to signing this document, should I become aware of a conflict of interest, actual or potential, that would require disclosure as set forth above, I agree that I will promptly notify the Boards of Advisors Office. If I conclude that a potential conflict of interest exists in relation to any transaction to be approved by the board or by the Trustee Executive Committee, I will recuse myself from participation in any decision relating to that transaction.
