Our research team at the Friedman School of Nutrition Science and Policy at Tufts University would like to invite you to complete a survey about cancer survivors’ eating habits. Your answers to the survey will help us provide better nutrition support to cancer survivors and families.

Your participation in this survey is voluntary. There is a risk of loss of confidentiality however this risk is minimized by not collecting names. Any identifying information you provide us (zip code, email address) will be kept confidential, used only for purposes of this research and only the research team will have access to this information. You will not be compensated for completing this survey; however your answers will help us design a nutrition program that best suits the needs of individuals who have survived cancer. Completion of this survey will be understood as your consent to participate in this survey. All answers are voluntary. If you don’t wish to answer a specific question you can skip to the next.
